DOT/SAP,  Drug/Alcohol Evaluation

This is normally required after a DUI or legal charge, positive drug screen at work, school drug/alcohol rule violation, physician-requested alcohol assessment, or a safety-sensitive employee evaluation.  DOT/SAP evaluations are required for employees whose jobs follow regulations such as: 

• Airline - FAA

• Marine - USCG

• Pipeline & Hazardous Material - PHMSA

• Railroad - FRA

 

Initial evaluations are completed by a certified Substance Abuse Professional (SAP) within 48 business hours of your appointment. While a violation by itself may not be a sign of a true substance abuse problem, the court, employer, or school will often want a drug and alcohol assessment completed in a professional and timely manner.

 

A fee of $300 is required at the time of service (not covered by insurance).

 

 

 

Medical Clearance

A pre-surgical psychological evaluation is standard practice before a patient undergoes several types of surgery. These evaluations help the physician learn more about your individual situation and needs in order to provide you with appropriate care before, during, and after your medical procedure.   Surgeries requiring  evaluations include:

• Bariatric 

• Cosmetic Surgery

• In-vitro Fertilization

• Organ Transplant

• Pain Pump Insertion 

• PNS

• Spinal Cord Stimulator

• Stimwave

• Podiatry
